Information you provide: when you request an appointment through our Jotform-hosted form, you may share your name, phone number, preferred office, preferred time and a brief reason for your visit.

Information collected automatically: like most websites, our hosting provider records standard technical data such as IP address, browser type, pages visited and referring site. We use this in aggregate to keep the site secure and improve it.

AI concierge conversations: messages you type into the Ask Healthy Smiles assistant are processed to generate a reply. Please do not enter medical record numbers, insurance member IDs, Social Security numbers or detailed health history into the chat.

How we use information

To respond to appointment requests, confirm visits, answer questions, improve our website and comply with legal obligations. We do not sell or rent your personal information, and we do not share it for cross-context behavioral advertising.

Service providers

We rely on trusted vendors to host this website, power our online forms (Jotform) and run our AI concierge. These providers may process information only on our behalf and under contractual confidentiality obligations. Where a vendor handles protected health information, we require a Business Associate Agreement.
